DESCRIPTION OF SERVICESPhysical TherapistJob Description 1099 Independent Contractor: We are looking to bring on a Physical Therapist for our location at NeuroRestorative, 1540 Teal Club Rd., Oxnard, CA 93030-8639 to support our residents. Must have a PT license for the Sate of CA and Liability Insurance. Please contact Becky Dutzy (805)-382-1921 to apply.Physical therapist will provide services that help restore or maintain function, improve mobility, relieve pain, and prevent or limit permanent physical disabilities of residents suffering from injuries or disease.When the physician orders physical therapy services, the therapists responsibilities shall include but are not limited to the following:Initial resident assessment, completed and documented within 72 hours of admission. Update quarterly, at a minimum, and as needed.Develop an individualized plan of care tailored to each resident's needs upon admission to include long and short term goals. Update quarterly, at a minimum, and as needed.Assess and treatment of range of motion deficits through exercise, positioning, and splinting.Tone assessment and use of muscle tone management, when appropriate.Assess and treatment of strength through therapeutic exercise.Assessment of balance, coordination, and gait with training in use of appropriate assistive devices.Use electrical stimulation, hot packs or cold compresses, and ultrasound to relieve pain and reduce swelling. Deep-tissue massage to relieve pain and improve circulation and flexibility.Orthotic/Prosthetic assessment and training program.Wheelchair assessment, transfer training and bed mobility intervention.Wound care evaluation and intervention.Assessment of home safety needs prior to discharge, when appropriate.Determine proper equipment usage, application and body position.Teach residents to use assistive and adaptive devices.Educate residents, family members and staff regarding impairments, disease processes, and treatment strategies.Monitor and document residents progress and adjust/modify physical therapy treatment/program accordingly.Provide appropriate referrals for related services.Collaborates with other staff members of the interdisciplinary team, as appropriate, for case management.Participate in the Interdisciplinary Team Conference, as appropriate, and report on resident status, goals and interventions.Participate in resident community reintegration and discharge planning, as appropriate.Participate in Performance Improvement Program, as appropriate.
